Gamewell-FCI Recalls to Upgrade Software Used with Fire Alarm System Control Boards

Hazard: Incoming and outgoing messages to and from the control board can interfere with each other, preventing it from sending a message to the command center in the event of fire.

Incidents/Injuries: Gamewell-FCI has received one report of failed communications during an electrical short that was not due to a fire. There have been no reports of property damage or injury.

Description: The software upgrade involves Gamewell-FCI model ILI-MB-E3 and ILI-S-E3 loop control boards. These boards are components of the Gamewell-FCI E3 Series fire alarm system. They provide the signaling line circuits and display information for the system.

Sold through: Authorized distributors nationwide from October 2005 through September 2006. Authorized distributors resell and install loop control boards for commercial end users in facilities such as hotels and office buildings.

Remedy: Gamewell-FCI is contacting customers directly and is providing a free firmware software upgrade.

American Honda Motor Corp. Recalls All-Terrain Vehicles for Crash Hazard

Name of Product: Honda Model Year 2006 TRX450ER/R ATVs

Units: About 11,000

Distributor: American Honda Motor Corp. Inc., of Torrance, Calif.

Hazard: The front suspension arm ball joints could have been contaminated during production, resulting in rapid wear of one or more of the ball joints and possible ball joint separation. If the ball joint separation occurs while riding, the operator could lose control of the ATV.

Incidents/Injuries: American Honda Motor Corp. has received seven warranty claims about this problem. No injuries have been reported.

Description: The recall involves 2006 model year TRX450ER and TRX450R ATVs. They are adult-sized ATVs designed for use by riders age 16 and older. The ATVs are available in red or black. The TRX450ER and TRX450R model names are located on the front cowling just below the handlebars and the name Honda is printed on the seat.

Sold by: Honda dealers nationwide from September 2005 through December 2006 for between $6,600 and $7,000.

Remedy: Consumers should stop using these recalled ATVs until the repair has been completed. Call any Honda ATV dealer to make an appointment to have the ATV repaired. The dealer will inspect and repair your ATV, if necessary. Registered owners of the recalled ATVs have been sent direct notice.

Old Williamsburgh Candle Corp. Recalls Mason Jar Candles Due To Laceration and Burn Hazard

Manufacturer: Old Williamsburgh Candle Corp., of Brooklyn, N.Y.

Hazard: The wick can move from the center of the jar to the side causing the glass to overheat and possibly crack or shatter. Exposure to broken glass and molten wax poses laceration and burn hazards to consumers.

Incidents/Injuries: Old Williamsburgh Candle Corp. has received 11 reports of the candles cracking or shattering, including one laceration injury.

Description: This recall involves all scents and colors of Mason Jar Candles in 3.5, 4.0, 4.5, and 5.0 oz. sizes. The size of the candle is specified on the label at the bottom of the jar, on the last line of the label. Replacement Mason Jar Candles, which are not included in the recall, have a “Q” on either side of the label.

Sold through: Dollar stores, discount stores, and grocery stores nationwide from May 2006 through December 2006 for between $1 and $1.50.

Remedy: Consumers should immediately stop using the candles and return them to the place of purchase for an exchange or full refund.

CPSC Warns of Deadly Fire and CO Dangers

WASHINGTON, D.C. -- With winter storms and cold weather impacting much of the country, the need for supplemental heating is on the rise. Continued reports of deaths and injuries associated with alternative heating products prompts the U.S. Consumer Product Safety Commission (CPSC) again to warn consumers to exercise extreme caution when using space heaters, kerosene heaters, and fireplaces.

On January 17, a 1-year-old, Westmoreland, N.Y., girl was killed when a space heater in her room ignited a fire. Her mother, while trying to rescue the child, received burns to her hands, arms and face. In another recent incident, a Tarrant City, Ala., couple was killed when embers from their fireplace ignited a fire that spread throughout the home.

“There are nearly 25,000 fires and 140 deaths on average each year from portable heaters, fireplaces and chimneys,” said CPSC Acting Chairman Nancy Nord. “Use these products properly and have working smoke and carbon monoxide alarms to help keep your family safe this winter.”

Home heating equipment is one of the most common causes of residential structure fires, second only to cooking fires. Portable heaters, including space heaters, are the leading cause of deaths in home heating equipment-related fires. Space heaters can cause fires if they are placed too close to flammable materials such as drapes, furniture or bedding. Fireplaces can cause fires if the chimney is cracked, blocked or coated with creosote, or if sparks and embers reach flammable materials.

Also, carbon monoxide (CO) from gasoline-powered generators that may be used during winter weather-related power outages can kill in minutes. Consumers should never use a generator, charcoal or gas grill in an enclosed area. In addition, fuel-burning appliances can cause carbon monoxide poisoning if they are improperly installed, poorly maintained, have defective or blocked venting systems, or are misused.

To help prevent deaths and injuries, CPSC urges consumers to:

* Have your heating system, water heater and any other gas, oil, or coal burning appliances serviced by a qualified technician every year.
* Install battery-operated CO and smoke alarms in your home. Locate CO alarms outside the bedrooms in each separate sleeping area. Locate smoke alarms on each level of the house and inside every bedroom.
* Replace smoke and CO alarm batteries in the spring and fall when you change the time on your clocks.
* If an alarm sounds, leave your home immediately and call 911.
* Seek medical attention immediately if you are feeling dizzy, lightheaded or nauseous. These are symptoms of CO poisoning.

Fireplaces:

* Have flues and chimneys inspected before each heating season for leakage and blockage by creosote (an oily deposit that readily ignites) or debris.
* Open the fireplace damper before lighting the fire and keep it open until the ashes are cool. Never close the damper if the ashes are still warm. An open damper may help prevent build-up of poisonous gases inside the home.
* Store fireplace ashes in a fire resistant container and cover it with a lid. Keep the container outdoors and away from combustibles.
* Never use flammable liquid to start a fire.

Space heaters:

* Place the heater on a level, hard and nonflammable surface (such as ceramic tile floor), not on rugs or carpets or near bedding or drapes. Keep the heater at least three feet from bedding, drapes, furniture and other flammable materials. Keep children and pets away from space heaters.
* To reduce the risk of fire, NEVER leave a space heater on when you go to sleep or place a space heater close to any sleeping person. Turn the space heater off if you leave the area. Never use extension cords to power electric heaters.
* Use a space heater that has been tested to the latest safety standards and certified by a nationally-recognized testing laboratory. These heaters will have the most up-to-date safety features. A newer gas space heater that meets current safety standards will shut off if oxygen levels fall too low.

Charcoal grills:

* NEVER burn charcoal inside of homes, vehicles, tents, or campers. Charcoal should never be used indoors, even if ventilation is provided.

Sally Foster Inc. Recalls Tea Lights Candles for Fire Hazard

Name of product: Tea Lights Sold with Votive Candle Holders

Hazard: The tea light candles have a clear, plastic shell that can melt or ignite, posing a fire or burn hazard to consumers.

Incidents/Injuries: Sally Foster has received two reports of the plastic shells of these tea light candles igniting, causing minor property damage. No injuries have been reported.

Description: The recalled tea lights were sold as part of the three-piece Glass Candle Holders with Tea Lights Set, item number S106 in the Sally Foster catalog and Web site. The product’s packaging was marked “Votive Holders with Tealights-Set of 3” and “Item Number 2006 157.” Only the white tea light candles with clear plastic shells are affected. Products delivered after December 8, 2006 included tea lights with metal shells and are not included in this recall. The glass candle holders sold with these tea lights are not subject to the recall and consumers may continue to use them.

Sold through: Sally Foster’s Web site from July 2006 through December 2006 and in Sally Foster catalogs from August 2006 through December 2006 for about $15.

Remedy: Consumers should immediately stop using the tea light candles and contact Sally Foster for a set of six free replacement tea light candles with metal shells. Consumers who purchased the item online will be directly notified by Sally Foster and will receive the free set of six replacement tea light candles.

TAC LLC Recalls “Erie Boiler Boss” Boiler Controls for Scald Hazard

Hazard: These boiler controls can fail, causing water temperature to rise to the high temperature limit. Should the high temperature thermostat or external safety limit devices also fail, consumers could suffer scalds from unexpectedly hot water during use or system piping damage can result.

Incidents/Injuries: TAC is aware of 17 incidents where the boiler control reportedly failed. One incident of system pipe damage has been reported. No injuries have been reported.

Description: The recall includes all Erie Boiler Boss 2400 Operating Controls and Erie Boiler Boss 1200 Boiler Reset Controls. The controls are typically used to cycle the domestic or hydronic water system burner. The control is mounted in an enclosure mounted on or near the boiler. The recalled controls have a nameplate on the front of the enclosure with the system name and model number. A numerical LED display will be part of the nameplate. Affected BB2400 carton labels on uninstalled products will have date codes prior to 0523. Affected BB1200 carton labels on uninstalled controls will have date codes prior to 0647. No date code is stamped on installed products.

Sold at: TAC wholesalers, independent field offices and original equipment manufacturers nationwide. The controls for the 2400 model was sold from February 1998 through June 2005, and the controls for the 1200 model was sold from July 1999 through November 2006 for between $550 and $650.

Home Decorators Collection Recalls Storage Trunks Due to Entrapment and Suffocation Hazards

Hazard: The trunk has an exterior clasp that can lock unexpectedly when the trunk lid closes. If a child climbs inside the trunk, he/she may not be able to open the trunk from the inside. This poses an entrapment and suffocation hazard to children.

Description: The recalled trunks are leather bound and have a handle on each side, and a clasp on the front. The trunks are burgundy or brown and are sold in three sizes: small, medium and large. The following catalog numbers were used to identify the product:

Flojet Recalls VAC Pumps Sold With LeBleu 50-Gallon Bottled Water System for Shock Hazard

Hazard: A manufacturing defect can cause consumers to receive an electric shock when the metal housing is touched.

Description: The recall involves Duplex II 115 VAC Pumps sold as a component of the LeBleu Automatic 50 Gallon Bottled Water System. Model numbers included in this recall are D08X000A and D08X000B. A silver Flojet label that includes the model number is affixed to the front of the pump. The pumps are installed exclusively in 50 gallon bottled water that were sold, installed and maintained by LeBleu Corp. or its licensees.

Sold through: LeBlue Corporation directly and through its licensees from June 2004 to November 2006.

Remedy: A representative of the LeBleu Corp. or a trained licensee will inspect all installed pumps to determine if the defect is present, and, if so, the pump will be replaced. All products installed in new home construction, which have not yet been started up will be removed and replaced.

Hair Dryers Recalled by Version-X Due to Electrocution Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of product: Travel’N Baby Mini Hair Dryers

Units: About 18,000

Importer: Detour Corp., doing business as Version-X, of Studio City, Calif.

Distributor: Metropolis Beauty Inc., of Los Angeles, Calif.

Hazard: These electric hair dryers are not equipped with an immersion protection plug to prevent electrocution if the hair dryer falls into water. Electric shock protection devices are required by industry standards for all electric hand-held hair dryers. If the hair dryer falls into water during use and is not equipped with this safety device, it can pose a shock and/or an electrocution hazard to consumers.

Incidents/Injuries: None reported.

Description: The recalled hair dryers are made of plastic and are red, blue or green. The hair dryer’s handle folds up. The words “Travel’N Baby” are printed on one side of the hair dryer.

Sold by: Independent beauty supply stores and beauty salons nationwide and online at www.metropolisbeauty.com from January 2004 through January 2005 for about $20.

Remedy: Consumers should stop using these hair dryers immediately and contact Version-X for a replacement hair dryer equipped with an immersion protection plug.
